• Proof of age (certified birth certificate or other document approved by the principal)
• Court order for guardianship (if student is living with someone other than the parent)
• Proof of residency (two documents)
• Immunizations/shot record and Kindergarten Health Assessment (See below for new immunization requirements)
• Early registration will ensure bus transportation on the first day of school
