    Sports Physical Forms

    Dragon Fly Guide

     All students interested in playing sports this coming school year, 2024-2025, should create a profile for adding their info, a current sports physical, and completing concussion paperwork. Updated Physical forms should be uploaded into the Dragonfly App or website at https://www.dragonflymax.com/parents . When registering your child, please select the sport(s) of interest and eighth grade regardless of their actual grade for dragonfly purposes, for example, "Softball 8th-grade".
     
    BStudents without a physical will not be able to participate in workouts or tryouts. 
    Tryouts are Monday - Tuesday, February 10th - 11th - 3:40- 5:30
    Students must attend BOTH days and have a ride pick them up at 5:30 each day. 
    Students must use their STUDENT School Google Account to access and complete registration. 
     
    Parents should plan to pick up their child on time each day at the back of the school beside the gym. 
     
    Tryouts are closed to parents/guardians, relative,s and friends.
