See below for how to attain the Hepatitis B Vaccination
1st - Please read the Consent and Release for Hepatitis B Vaccination form. Please sign as either accepting or declining the vaccine and return this form to Mike Whitford at Facility Services.
2nd - If you are accepting the Hepatitis B Vaccination, contact the Pitt County Health Department (252-902-2449) to establish an appointment for the administration of the vaccine.
3rd - Take your Hepatitis B Vaccination Authorization (we will send to you) with you to the Pitt County Health Department on the date of your appointment. The Hepatitis B Vaccine will not cost you anything. However, without the Hepatitis B Vaccination Verification the Pitt County Health Department will not administer the vaccine.
The vaccine is a series of three shots over 6 months followed up with a post vaccination serology test. Prior to the second, third, and fourth appointments, you will need to contact Mike Whitford to receive a new Hepatitis B Vaccination Verification. The Pitt County Health Department requires a new verification form for each visit.
